Members
North Carolina’s Regional Councils of Governments (COGs) provide or contract for social supports that address health-related social needs through a coordinated network of community-based organizations.
In December 2023, 12 of 16 COGs formed the North Carolina Community Care Hub to provide statewide coverage in order to coordinate contracting with health plans, hospitals, affordable care organizations and more for the delivery of health-related social needs to their members and patients.

From Murphy in the west to Manteo in the east, providing connections in all 100 counties.
